    Therapeutic Dietary Assessment

    Nutrition is a key component for a healthy lifestyle but yet requirements differ in older adults due to the physiological changes as people age. Many seniors and adults with complex medical issues have dietary restrictions from low-sodium, low-sugar, to diabetic diets. Maintaining a proper diet helps to lower cholesterol, lower blood pressure, promote more energy, and cognitive health to name just a few of the advantages.

    Our Registered Dietician will meet with you and will collaborate with the team to fully learn your medical history and other facets of your life to assess. She will then make recommendations to develop a  healthy and customized menu that ensures proper nutrition is being met.

     

    Based on your needs we can also shop for your loved one, provide recommendations for personal chefs or delivered meals catered around your specific needs.

    Recreational Therapy

    Recreational Therapy and Activity Programming is a supportive approach that utilizes recreation and other activity-based interventions designed to meet the needs of older adults and those with Alzheimer's Disease and Related Dementias (ADRD).

    ​

    This adjunct service to care management is a holistic program that promotes the six dimensions of wellness: Intellectual, Physical, Emotional, Occupational, Social and Spiritual pursuits.

    ​

    A qualified staff member will work to provide a specific program designed to meet your loved one's own interests and will work with community resources to offer the widest range of options available for your loved one.

    Supportive Services

    In order to stay as independent as possible, we can provide a vast array of services to help support your loved ones: 

    • Safety evaluations to identify obstacles and provide solutions to living independently

    • Arrange, and monitor in-home help

    • Accompany clients to appointments 

    • Arrange transportation for appointments, errands and social activities

    • Provide referrals for home maintenance

    • Assist with bill paying, shopping, de-cluttering 

    • Coach and educate family and caregivers

    • Arrange delivery and installation of durable medical equipment

    • Provide an objective critique of residential care facilities

    • Arrange placement in Senior Care Community (retirement living, independent living community, assisted living community, personal care home or skilled nursing facility)

    • Coordination of downsizing or move to Senior Housing Community 

    • Onsite monitoring of care communities, attend care/service plan meetings and monitor compliance and quality
